Criteria for Admission

Individuals receiving services from Uniquely You Services, LLC shall meet the following criteria for admission:​

 

An individual must have a diagnosis of developmental disabilities as defined in the Code of Virginia disabilities like Autism, Cerebral Palsy, Asperger’s Syndrome, Pervasive Developmental Disorder, Traumatic Brain Injury, Muscular Dystrophy Spina Bifida, Spinal Cord Injury, Shaken Baby Syndrome, and spinocerebellar degeneration or have other documented needs in two or more adaptive skill area.

 

The individual must also have an IQ of below 70 testing often comes from a licensed psychologist (through school, insurance, or private pay) and should be no more than a few years old.  If an individual is over the age of 22 when they first were diagnosed or when the test is re-done.

 

The individuals will need to show documentation from before the age of 22 indicating a disability was present (e.g., an IEP, or a statement from a doctor).

 

​An individual must meet the level of functioning eligibility criteria in two or more of the following areas: health and safety, communication, task learning, personal self-care, mobility, behavior management, or community living skills.​

 

An individual must have had an SIS assessment completed or scheduled which would identify the individual's Tier and Level of Support. If an individual does not have a current SIS assessment but has one schedule and desires to move into one of the homes after receiving all the admission and upon review, a decision shall be made by the CEO/Executive Program Director and/or designee to whether the company would be able to provide quality person-centered care to the individuals.
